Hi Dears,

Please assist to correct my name on my vaccination details. As per checking my first and second dose were updated without "Jr".

But my booster or first additional shot is updated with "Jr."

Details in my vaccine card has no Jr as well, but form filled up has JR.

No Vax Cert was generated yet due to different names in site. One with Jr. And other one,without Jr.

You asked for Vaccine Card.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
