Hi! My name is Julie keng I would like to request the VaxCertPH online form be updated so I can include the covid booster shot, Moderna, I received on Jan. 14,2022 (NPF Drive Through Batch/Lot Moderna 010G21B).

I need to present it when I travel abroad on Oct 15 2022. Right now on Vaxcert online It only allows to enter the first dose (June 16, 2021 -Sinovac  B202104046 at Manila) and second dose  (July 14, 2021 -Sinovac L202106034 at Manila) of primary vaccine received.

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may call the Vax Cert
Customer service Hotline. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
