Greetings!

I am Lloyd Ian M. Capiz, and I'm writing to request for an urgent portal update of my vaxcert for my 2nd Dose of vaccine is not recorded in your system.

I have already updated my info in the portal and I wanna know when will I be able to print my certificate. The system shows the request is already completed but I still cannot print it and tells me to wait still. 

I have a flight on October 03,2022 and I badly need my vaxcert to be released asap. I'm hoping for your help and consideration on this matter. Thank you very much

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
