Dear FOI,

I  would like to have my Vaxcert updated in the system as my second dose is not recorded. I will be needing the vaxcert to travel local and abroad.

Below is my vaccination details:

First dose( Pfizer) - 05/02/2023
Second dose( Pfizer)- 05/23/2023 (no record)

Response to Request

Your FOI request is approved. 

Please be informed of the following options on how to avail of acceptable proof of COVID-19 vaccination in the meantime while VaxCertPh is currently experiencing Technical Issues:

1.	Check if your VaxCertPH is available at https://vaxcert.doh.gov.ph. If you can generate your VaxCertPH with no errors or missing information, you may use this certificate as proof of COVID-19 vaccination. 

2.	You may use your LGU vaccination card or International Vaccine Certificate (ICV). Kindly refer to the attached DOH Memorandum Circular No. 2022-0051 as your reference.

Your right to request a review
