I would like to inquire how to get a vaccine card, mt sister just had completed her vaccine yesterday (05/12/2022). However, this morning our dog bitten it to pieces, by the way my sister is 11 years old.
May I ask if how could I get new vaccine card for her?

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
