Good Evening

I would. Like to request for un update in the information of my Vaxcert
I had a vaxcert however on record Only the booster was placed on the card

I have been vaccinated on the following schedules
1) March 17,2021 - 1st dose- Astrazeneca
2) June 7,2021 - 2nd Dose - Astrazeneca
3) 1st Booster dose- Dec 1,2021- Pfizer

*all doses were administered in the City of Calamba, Laguna

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may call the Vax Cert
Customer service Hotline. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
