Hi! Good day! To whom it may concern, I am writing this letter for the request of financial assistance due to contraction of Covid-19 last August 25, 2021 and has been advised to quarantined up to September 9, 2021. I am a Administrative Assistant II working in the Lung center of the Philippines and would like to make this humbly request, may I know if I am qualified and can send the required documents.
