Good day! I am a medical technology student studying at the University of the East Ramon Magsaysay Memorial Medical Center, Inc. I would like to request information (for an activity in our course, HISMLS) regarding the following: 

1) Name of PHC's HMIS provider
2)  Year PHC acquired or began using their HMIS
3) PHC hospital departments that are utilizing HMIS
