Hi! In order to replace my missing vaccination card, I would want to request another one. I don't have any images of my vaccination record, but I do recall where and when I received my shots. Under the name Justine Balo, I received my first dose on October 29, 2021, and my second dose on November 19, 2021, both at the Gaisano Mall of Tagum. I'm hoping my request is granted because I'll need it for my enrollment the following week.

While our aim is to provide information whenever possible, in this instance this Office does not have the Covid-19 Vaccination Card, the information you have requested. However, you may wish to contact <Tagum City Health Office> at <Ernesto Punzalan Street, Tagum City, Philippines near Magugpo South Barangay Hall>, or resubmit your request addressed to the correct agency on the eFOI portal.
