Hi! I would like to request tabulated data (excel file or any format) regarding the number of crash incidents (e.g. vehicular accidents, involving pedestrians) in UP Diliman including the following:

1. Time and date of incident
2. Location (Street/Avenue/Road)
3. Collision type
4. Factors/reason leading to the incidents
5. Number of injuries/deaths
6. Vehicle type involved

I already sent a notarized certification of proof of citizenship and a gov't ID indicating my citizenship (passport) to fro.updiliman@up.edu.ph as required by the UP FOI Manual.

Thank you for your request dated Feb 24, 2019 08:28:29 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Number of crash incidents at UP Diliman campus.

Response to Request

Your FOI request is approved. I enclose a file prepared by the UP Diliman Police containing information about vehicular incidents that had been reported to them. We have removed the names of the victims/complainants and respondents, and replaced them with P1, P2, etc., where applicable.

This is to respectfully remind you that the information provided to you by U.P. must be processed (used)  only for the purpose/(s)  stated in your request pursuant to the provisions of E.O. No. 2 Series of 2016, R.A. 6713 and its Implementing Rules and Regulations, the Philippine Data Privacy Act and other applicable laws.  The DPA which is a criminal statute imposes the penalty of imprisonment and a fine for the unathorised processing of personal and sensitive personal information.

We request you to kindly acknowledge receipt of this letter and that you understand that you are bound to comply with the provisions of, among others, the Philippine Data Privacy Act in using the information you requested and that you will hold the University free and harmless from all liabilities in the event that you process the information provided in violation of applicable laws.
