Hi, I would like to request for the reissue of my vaccination card because I lost it due to typhoon Odette.
 I lost it because house was vanished by flood. I'm looking forward for your consideration.I need it to enroll myself to your univesity. I'mm looking forward for your response. Thank you.

You asked for REQUEST FOR RE ISSUE OF VACCINATION CARD.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have a copy of your vaccination card,  the information you have requested, because WPU is not the right agency/office to issue/re issue a vaccination card. However, you may wish to contact your LGU/RHU for the copy or for  re issuance of your card where you had your vaccination, or you may  resubmit your request addressed to the concern LGU you are currently residing on the eFOI portal, if your LGU is already a member of EFOI portal.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Prof. Merline B. Rabaya, Information Officer of WPU. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
