I am writing to request a copy of my booster vaccination certificate for the COVID-19 vaccine. I recently completed the recommended interval for the booster dose after receiving my initial vaccination earlier this year.

As I plan to travel internationally, it is essential for me to have the necessary documentation, including the booster vaccination certificate, to comply with entry requirements. Could you please provide me with an official booster vaccination certificate via email in PDF format?

Greetings!

Thank you for your request dated May 15, 2023 07:23:53 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for a request for booster Vaccination Certificate.

Ms. Villaruel be correctly informed that requests related to issuance of Vaccination Certificate/Card is a function of the Department of Health through its accredited clinics and hospitals.

The Overseas Workers Welfare Administration is mandated by law to provide welfare assistance and benefits primarily to OFWs in distressful situation.

Hence, it is strongly suggested that you visit the DOH website on your concern or coordinate with the clinic/hospital that provided the vaccination shots.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing the OWWA- FOI Decision Makers at email: foi_dm@owwa.gov.ph. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.

However, if you are still not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
