Hi i would like to get my vaxcertificate  but i didnt trace my record using my peesonal detail can i use my QR to trace it

Dear Ms. Nor asmiya Mohammad,

Greetings!

Thank you for your request dated Jan 28, 2023 03:47:44 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch a copy of yours vaxcertificate with QR code.

Ms.  Mohammad be correctly informed that the Overseas Workers Welfare Administration (OWWA) is not mandated nor authorized by law to issue vaccination certificates.  The issuance of vaccination certificates is the function of the Department of Health thru its accredited Medical Clinics.

Hence, OWWA suggests that you submit your requests to the Department of Health or go to the clinic/LGU Health Center where you got vaccinated.

Moreover, the Freedom of Information is not intended to respond for requests for personal data or information as it violates the Data Privacy Law. The QR Code is a personal information.

 If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing the OWWA- FOI Decision Makers at email: foi_dm@owwa.gov.ph. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.

However, if you are still not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
