May I ask for the template of the "Bidder's Statement of all Ongoing government and private contracts, including contracts awarded but not yet started"?
Please also include the instructions on how to accomplish the template.

Thank you for your request dated March 23, 2022, under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.
 
Your request 

You asked the Government Procurement Policy Board-Technical Support Office (GPPB-TSO), for research purposes, for a template, and instructions to accomplish, the Bidder’s Statement of all Ongoing Government and Private Contracts (“Statement”). 


Response to your request 

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

Please note that Section 23.1(a)(iii) of the 2016 revised Implementing Rules and Regulations of Republic Act No. 9184 which provides for the submission of the Statement did not provide a specific template for it.  Thus, the Procuring Entity may consider any format, so long as the information of all ongoing, completed, and awarded but not yet started contracts with government and private entities regardless of similarity in the nature or complexity of the contract to be bid, within the relevant period stated in the Bidding Documents, will be indicated in the form.

Bear in mind that the purpose of requiring the Statement is to gauge the procurement experience of the Bidders and their awarded procurement projects, to show their aptitude and capacity to deliver, do or perform the contract to be bid.   

In the absence of a pro forma form of the Statement under Rep. Act No. 9184, you can check the templates provided by other agencies which are uploaded in their websites, as follows:
