Dear MR. JOHN MICHAEL D. SANTIAGO,

This refers to your eFOI request received on 13 October 2022 sent to the eFOI portal of the Home Development Mutual Fund (Fund) wherein you inquired whether a member may submit a scanned copy of the membership form, via email or online, for the amendment/update of his/her details. 

Our Office deemed it proper to raise this matter to and confer with the operating unit concerned – the Member Services Sector Support Unit (MSSSU).

Kindly be informed that pursuant to the Fund’s Procedure on Updating of Member’s Record, (Document No. HQP-PF-019), a member submits the duly accomplished Member’s Change of Information Form (MCIF) and supporting documents, if applicable, to any of the following:
1.	Member Services Office (MSO)
2.	Branch
3.	Servicing Department/Loans Origination – Housing Business Center

If the member cannot submit the MCIF personally, the authorized representative must submit/present the following documents for validation:
1.	Authorization Letter duly signed by the member
2.	One (1) valid ID of the member and authorized representative
