Hi! Recently I found a PIDS study (DP 2023-01) which used the PNHES dataset.  To better understand how the responses in the PNHES were encoded to the dataset, I would like to request a copy of the questionnaire used for the survey. This will be very helpful for the research paper that I am writing.

However, you may want to get in touch directly with the Health Policy Development and Planning Bureau (HPDPB) of the Department of Health (https://doh.gov.ph/) since the DOH is the owner of the PNHES dataset.
