Hi! I am humbly asking for a replacement copy of my vaccination card since most details are not readable now and I badly needed it for university. I wish to have both a digital copy and a physical one. 

I requested this via email and they only replied that it will be referred to the concern office. After a period of time, I no longer receive an update if my concern was referred. I am hoping to get a feedback from you.

Your FOI request is approved. We enclose a copy of a Vaccination copy in PDF form with certificate. It is ready to print.
