Good Day!  My Father and Mother got their Covid Booster shots in Veterans Memorial last November/December 2021.  They don't remember the exact date.

They are both Senior Citizens and are intending to get the 2nd dose of Booster Shot since they are already eligible. however, they cannot find the piece of paper given to them as proof that they already had the first dose.

Hope you can assist us in obtaining these information.

Dear Maria Patricia Abogado,

I refer to my letter of May 25, 2022 06:36:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for Covid Booster information.

In order for us to work further on your request, we need more information from you regarding your query such as the following:
                  -Full name of patient
                  -Date of birth
                  -CP #
                 -Address
                 -Brand of vaccine
                 -Date of 1st dose
                 -Date of 2nd dose
                -Date of 1st booster
                 
This info is BOTH for your parents for us to retrieve the info since you said they cannot find their vaccine cards.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 18, 2022 12:44:29 AM, your request will be closed.
