I would like to request a schedule for a second booster. I've been going in and out of the hospital due to check ups and laboratories. Please schedule me ASAP. Thank you for your kind consideration!

Please drop by on June 24, 2022 for the vaccination schedule. Or we can send to you the QR code prior to the said date. Also,  we have an email to you to  provide us details on the lost vaccination card. These info are needed if you want a replacement vaccine card.
