September 6, 2021 Dear Alianie Pearl, Greetings! Thank you for your request dated Sep 02, 2021 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Covid positive. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have all the information you have requested. However, if you are an employee who suffered from a work-connected COVID, pls file your EC sickness claim at the SSS branch nearest you. The forms are available at the SSS/ECC website. EC Claims Requirements: 1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19. 2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center 3. Medical Certificate or Home Quarantine Clearance issued by LGU Health Office/BHERT 4. 2 Valid IDs Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report) If you already have an approved EC sickness claim at SSS, you can then file an EC cash assistance claim at the ECC. The EC CA application form is available in our FB page and the requirements are as follows: 1. Proof of Approved EC Claim Voucher from SSS 2.
