Response to Request

REQUEST FOR CAV

-	To request for CAV, you may personally visit the Registrar’s Office from Monday to Friday 8:00AM to 4:00PM and provide a photocopy of your Transcript of Records and Diploma for authentication purposes. Present the copy of your TOR and Diploma at the Registrar’s window and pay Php 50.00 at the Cashier’s Office. All requested documents are processed within 7 to 10 working days upon receipt of the necessary requirements and payment.
