Hi! I would like to request information regarding my vaccination record. I lost my vaccination card last February. I check my record through vaxcert.doh.gov.ph but it only shown my second dose. There is no record of my first dose.

I was vaccinated in the Hiyas convention at Malolos,Bulacan. My first dose was on November 24 2021 and my second dose was on December 20, 2021
