Good day! I'm Roger Jr. D. Balanay, 25 years old, living in Union, Claveria, Cagayan. I graduated at Cagayan State University-Sanchez Mira Campus with the degree of Bachelor of Secondary Education Major in MAPEH with a Student ID No. 14-00116.

I would like to inquire if there is a possibility that i can request for a correction of my College Diploma particularly in the full name. The name in the diploma I was given to is written as ROGER D. BALANAY, JR. which should be in this format as ROGER JR. D. BALANAY.
