          My name is Arlene Adlawan Tulang, I am a graduate student of ALS way back 2008 at Mintal Comprehensive High School, I take my ALS Accreditation and Equivalency Test Secondary Level last Febuary 2008 at Daniel R. Aguinaldo National High School with number 17369. I would like to request for a Certified Original Copy of my ALS Certificate.I hope you will grant my request. Thank you and God Bless!

For requests on the issuance/re-issuance of Test Certification or Certificate of Ratings (COR) in the following testing programs:
 
-Philippine Educational Placement Test (PEPT)
-Accreditation and Equivalency (A&E) Test
-National Career Assessment Examination (NCAE)

Please download the COR Request Form here: http://bit.ly/CORRequestForm and fill-out the document. To expedite the verification of test results, kindly provide all the necessary details in the form then upload the COR Request Form and other relevant requirements through this link: https://tinyurl.com/BEACORVerify.

Please wait for DepEd-BEA’s email regarding your request at least two weeks after filing.
