I Mark Anthony Valbuena , would like to make a request copy of my form 137 last1996 or 1997 in La Huerta National High School as I am not able to remember the exact date due to its has been long time and only now I am needed it for my educational status. However upon searching for my form 137 in the school district where I studied, I found out that the school is no longer operating hence I’m seeking from your reputable and respectful department a help in order to get my Form 137

Response to Request
Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).

Do I need to go in my previous school and I can request my form 137? And they can give me a copy without any request coming from other school or deped?
