Hi, I would like to request my Form 138 copy for Grade 12 grades. School: Barretto Senior High School Reason: Nawala po kasi nung time na lumipat kami ng bahay yung card ko for Grade 12 and now College requirements are both Form 138 & Form 137 copy If I were to be approved permission to have a copy of form 138 a school from Barretto Senior High School ng grades for Grade 12, because some college require hard copy of Form 138. Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
