Hi !I am Manilyn Tayoto Diaz.
I would like to request for my NSTP Serial number from my previous school Southern Luzon State University Tagkawayan Quezon during 1st and 2nd Semester dated June 2006- April 2007.We tried to follow up this but they said they are still on the process
I badly needed this documents for Enrollment this coming January 13,2020
I am third year college student at San Mateo Municipal College.
My current school instruct us to have a letter being notarized and state that if we cannot provide this I will not graduate next year.Pls help me

We advise you to personally request your NSTP Serial Number to the Office of University Registrar and look for Ms. April/Ms. Joy or contact them thru phone: (042) 540 - 4763.  Please prepare the following: 

Requirement:  Letter of Request of your current school (San Mateo Municipal College) 
Fee:  100 Pesos
