Hi, I would like to request about the special order in my transcript of records with a course of Bachelor of Science in Civil Engineering. I only got my copy of records but I don't know that special order is needed. Also, I graduated at Southern Luzon State University, Lucban, Quezon last June 2018.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have copy of your request documents (transcript of record with special order).  Since, you were requesting for  personal information of the student, we advise you personally inquire to SLSU Office of the University Registrar fb page, email or contact them thru tel. no. 042-540-4763. 

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <Ms. Aurora L. Sumague through email . Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
