Hi,I am Kimberlyn Pacinos, I would like to request for my NSTP serial number from  Southern Luzon State University in Lucban, Quezon( school year: june2008-April 2009) for academic purposes. My current school need the record of my NSTP serial number from previous school for my graduation this year 2020 in line with the requirements of CHED.

While our aim is to provide information whenever possible, in this instance we advise you to personally request your NSTP Serial Number to SLSU Office of University Registrar and look for Ms. April/Ms. Joy or contact them thru phone: (042) 540 - 4763.  Please ensure to prepare the following: 

Requirement:  Letter of Request of your current school 
Fee:  100 Pesos

Your right to request a review
