In reply, your PARENTS must accomplish/ed the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:
- Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides; (1 original) - Barangay
- Duly Notarized Affidavit of Low Income/No Income; (1 original) - Taxpayer/Notary Public
- Proof of Payment of P100.00 Certification Fee and P30.00 loose Documentary Stamp Tax. (1 photocopy) - Collection Section of the Revenue District Office (RDO) having jurisdiction over the place of residence of the taxpayer.
Additional Documents, if applicable to the following cases: If transacting through a Representative:
Special Power of Attorney (SPA) executed by the taxpayer-applicant; (1 original); and
Any government-issued ID of the taxpayer and authorized representative; (1 photocopy).
Note: IDs shall be presented and should be readable, untampered and contains consistent information with the documents presented upon application.
Kindly submit the Certification of Low Income/No Income of your PARENTS to Revenue District Office (RDO) based on your residential address. You can retrieve RDO information on our website www.bir.gov.ph.
Your right to request a review
If you are dissatisfied with our response to your FOI request, you have the option to request an internal review. To initiate this process, kindly submit your written appeal for review addressed to [insert name of Secretary and postal/e-mail address]. Please provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response. We will conduct the review and communicate the outcome within 30 calendar days from the receipt of your review request.
Should you remain unsatisfied with the review outcome, you retain the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
