I am pleased to inform you that your FOI request has been approved. Please be informed that this data request is handled by select DOH Hospitals. We have inquired to DOH hospitals and was endorsed your FOI request to the following DOH Hospitals: 1. Tondo Medical Center 2. Valenzuela Medical Center 3. Philippine Orthopedic Center; and 4. San Lazaro Hospital .

We are still awaiting for the data from Philippine Orthopedic Center and San Lazaro Hospital. We shall upload the data on the said Google drive link once available. For data from private hospitals, we ask that you directly coordinate with private hospitals for this data request.

Your right to request a review
