For a research project on the Integrated Tuberculosis Information System, I am requesting:
1. A list of all entry fields for a patient's account when first enrolled and for any subsequent visits (in case those two are different); and
2. A list of all types of information that can be part of a patient's account. (Ex.: name, date of first admission, diagnosis, app logins)
