Age, sex, socioeconomic status, geographic location (region, province).
Primary diagnoses (ICD-10 codes) and comorbidities.

2. Symptom Data:
Type of symptom: (e.g., pain, nausea, dyspnea, fatigue, anxiety, depression, delirium).
Severity/Intensity: Measured using validated scales (e.g., numerical rating scale, Edmonton Symptom Assessment System (ESAS), or other relevant scales used in the Philippines).
Frequency and duration of symptoms.
Triggers and alleviating factors (if recorded).
Impact on daily activities and quality of life (if recorded).
Data Source: This might come from hospital records, specialized palliative care units, or potentially from patient surveys or assessments conducted by the DOH.
3. Medication and Intervention Data:
Medication name, dosage, frequency, route of administration for medications commonly used in palliative care (e.g., analgesics, antiemetics, anxiolytics, antidepressants).
Non-pharmacological interventions (e.g., relaxation techniques, psychosocial support, spiritual care) - if documented.
Changes in care plan (if available).
Response to interventions (if recorded).
Data Source: Hospital records, pharmacy records, or palliative care unit records.
4. Healthcare Utilization Data:
Hospitalizations: Dates of admission and discharge, length of stay, diagnoses and procedures during hospitalization (ICD-10 codes).
Outpatient visits: Dates of visits, services provided.
Emergency room visits: Dates of visits, reasons for visits.
Hospice care enrollment: Dates of enrollment and discharge (if available).
Data Source: Hospital records, Philippine Health Insurance Corporation (PhilHealth) claims data (if accessible), or other relevant DOH databases.
5. PhilHealth Data (if feasible):
Claims data related to palliative and hospice care: This could provide valuable information on healthcare utilization, costs, and potentially medication use.
