Hi. I would like to have my VaxCertPH RYUKIE R. NAJARO, BORN ON Davao City request to be reviewed and dealt with immediately. My 4th dose Vaccine was not recorded in my Vaxcert. It has been over almost a month upon request in the vaxcert.doh.gov.ph. I've already contacted through the e-mail provided in the link ([email protected]), but yet there's no response. We want it to be taken care of, ASAP, this is a requirement . Thank you.

We understand your concern however, please be informed that the encoding of vaccination records is done by the concerned vaccination center. If you want, you may kindly coordinate with the Local Government Unit (LGU) where you received your vaccination and request for assistance to generate your vaccination certificate.

Your right to request a review
