I would like to raise a concern regarding my vaccination record. I lost my vaccination card last Month. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. I was vaccinated in Bulan Freedom Park at Bulan Sorsogon. My first dose was on October 11, 2021 and the second dose was at November 03, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it.

July 19, 2022 Dear Juvylyn Fugnit, I refer to my letter of Jul 18, 2022 10:06:32 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Vaccination card. In order for us to work further on your request, we need more information from you regarding your query. May we please be informed if you have submitted a photo copy of your vaccination card in Sorsogon State University? If yes, in what particular office, for us to determine where to address your concern? During the period of us awaiting your reply, your request will be put on hold.
